Accessing Consent Forms

• Patients log into the patient portal and go to Messages to find the consent form link.

Completing Consents

• Patients review and agree to use electronic records and signatures, then sign and submit all required consent forms.

Confirmation

• After submission, a confirmation appears in the portal.

• Providers can view the completed consents in the patient’s profile, where documents are accessible via a link.
